PUD Adopted Public Records Request Rules

Our standard form is located on the link below. You can download, fill out and submit the required public records request form below by mail (link below) or submit electronically (see form at bottom of page).

If you are downloading the form and either mailing or emailing, please submit the request to our public records officer, Annette Johnson at ajohnson@jeffpud.org , call at (360)385-8351 or fax at (360) 385-5945. The form can be dropped off at our facility at 310 Four Corners Road.

Tort Claim

If you believe you have experienced damages due to PUD negligence, your situation may be reimbursable via a tort claim. However, the vast majority of outages are not due to PUD negligence. Most are unpredictable and unavoidable “Acts of God” such as windstorms, fallen trees, car accidents (downed pole), etc. that are beyond the control of the PUD and therefore are not eligible for claims.

We highly recommend you install whole home and individual circuit surge protection to protect your home and personal electronics from damage.

To submit a claim, please download and fill out the PDF form. You can fill in much of the form in Adobe Reader, but you must physically sign it. Please print and sign two copies, keep one for your records and submit one to Annette Johnson via email (via scan or good phone photo), fax at (360)385-5945 or mail at Tort Claim, c/o Jefferson PUD, 310 Four Corners Road, Port Townsend, WA 98368.
